Per-app and per-environment notification preferences

Notifications can now be configured at the environment and app level:

Slack: the shared app mute list is replaced by per-channel environment and app preferences, so different teams can route different apps or environments to their own channels.

Personal notifications: instead of being tied to whoever deployed a function, members can now follow the apps and environments they care about — via email or in-app — regardless of who (or which service user) deployed them.

In-app notifications: a new in-app notification center takes you straight to the affected resource in the dashboard.
